Black Hills Works proudly delivers South Dakota disability services, supporting people with intellectual and developmental disabilities for over 65 years. We deliver a wide range of services—including residential support, employment, transportation, community involvement, and arts expression—that empower people to thrive.
The first step toward admissions to Black Hills Works or another Community Support Provider is to contact Dakota at Home at [email protected] or by calling 1-833-663-9673 and pressing option 2 for the Division of Developmental Disabilities (DDD). Staff at Dakota at Home (not Black Hills Works) will provide the necessary application and paperwork to determine eligibility for services.
To qualify for services, individuals must meet certain following eligibility requirements, including:
- They must have been diagnosed with an intellectual or developmental disability before age 18. Qualifying conditions may include autism, Down syndrome, cerebral palsy, epilepsy, head injury, brain disease, fetal alcohol syndrome, and others.
- Financial eligibility also applies. Individuals must either receive Supplemental Security Income (SSI) or be determined to be “aged, blind, or disabled” with an income less than 300% of the SSI standard benefit amount.
Once South Dakota’s Dakota at Home staff determine that a person is eligible for services, the individual will be connected with a Conflict-Free Case Manager (CFCM). The CFCM will work with the individual and their support team to determine services needed and assist in connecting with the appropriate providers.
If an individual already receives services from a CFCM and is interested in transferring to the Rapid City/Black Hills area, the CFCM will assist you with a referral to a provider of South Dakota disability services, like Black Hills Works.
